Part I: Personal Particulars ^(1) (2)

⁽¹⁾ Applicants must enter the same particulars used at the time of their application.
⁽²⁾ Applicants may refer to the acknowledgement / confirmation email / email of online document verification arrangement for their Application ID.

Part II: Uploading of Supporting Document(s) ^{(3) (4) (5)}

⁽³⁾ Applicants should rename the supporting document with their Application ID as the prefix. (e.g. 2200001_AD Transcript)
⁽⁴⁾ Applicants may upload multiple files for each section.  However, files are limited to 5 MB each.
⁽⁵⁾ Applicant should upload their supporting document to the corresponding boxes (e.g. 3b for Transcripts and 3c for Public Examiniation Results).
⁽⁶⁾ The name of the uploaded file(s) will be shown below.  Applicants wishing to delete their submitted documents should click the red "X" next to the filename.
